Apply For a Fellowship Scholarship

Before You Begin

  • Your essay must be formatted correctly or it will not be processed.
    1. Please include your name at the top of your essay.
    2. Make sure it is a pdf or word doc. Pages is not acceptable.
  • Write an essay (not to exceed 500 words, subject is noted below) and save as a PDF, then upload below.
    1. Why do you wish to participate in the Fellowship?
    2. How do you intend to use the Integrative Medicine knowledge and experience gained in the Fellowship?
    3. How do you think this will impact your community?
  • Provide a letter of support and recommendation from the department head or director (or equivalent) of your practice location or program. If you do not have a department head or director, please ask a colleague or co-worker. Reference letters written by applicant or a family member will not be accepted. If you have any questions, please contact admissions. This letter should include:
    1. Their awareness and endorsement of your application in the Fellowship.
    2. Recognition that, if selected, completion of the Fellowship will require approximately 8 hours per week to complete the online instructional modules and associated readings. In addition, you will need to attend three 5-day Immersion Weeks in Tucson at the beginning, middle and end of the 2-year Fellowship. Please note that there is no requirement that this time be allocated by your worksite, however, our experience is that it is important for your colleagues or director to be aware of and comfortable with this additional workload.
    3. Documentation that you are in good standing within your clinical setting.
    4. Any other comments regarding your work performance and personal characteristics.

Optional materials to provide to the scholarship committee to demonstrate need or work with the underserved. One or more of the listed materials may be provided.

  1. Complete a FAFSA and upload your FAFSA Confirmation Page Email. A FAFSA allows the scholarship committee to compare need objectively. Your FAFSA is being used for evaluation purposes only, the center is unable to offer federal loans to pay for the Fellowship.
  2. Please include a short statement of need.
  3. Please include a short statement of how you treat the underserved. Please indicate in your statement if you work for the Indian Health Service, in a Federally Qualified Healthcare Center, or a Community Healthcare Center. If you are an international participant, please include a short statement outlining the underserved population you treat.
  4. Please include a statement of your involvement in indigenous healing traditions. This includes training in healing that is derived from an indigenous tradition or community from any area of the globe and/or be providing healthcare to an indigenous community. (Indigenous for this scholarship refers to healing traditions originally practiced by local healers in the culture it originally served.)
